Below is important information regarding parent and student drivers for away competition
Every parent driver must have a copy of the Parent Driver Form on file in Athletics along with copies of their driver’s license and current auto insurance card. Freshman parents must turn in their form here at DHS, not to their junior highs.
Students may only drive themselves to away matches if the coach agrees to allowing students not to participate in the team carpools. If so, then they must have a copy of the Student Driver Form on file in Athletics along with copies of their driver’s licenses and current auto insurance card. They may NOT drive other students.
Prior to the carpools leaving campus, a copy of the Team Carpool Driver Log must be on file with Athletics as well as with the coach. This log lists each driver, which students are in their car and parent emergency contact number.We have added the parent emergency contact info as well as emergency procedures for all drivers. This is essential to know in case of an accident or emergency during transport. Each driver must carry a copy of the carpool log so that they have access to parent emergency contact info and emergency procedures. All athletes are asked to carry a copy of their emergency card in their sports gear bags. Coaches also have all emergency cards. Car assignments must be completed prior to the departure of the carpool so that all parent emergency contact info can be complete. These logs can be brought to Athletics as hard copies or emailed to me. In a last minute situation, you can take a photo of the log and send it to Jeff’s email firstname.lastname@example.org but we prefer to have these logs ahead of time to make sure that all drivers are compliant.
